Exam Application
Instructions for Completing the Application Form
- It is important that your application is completed carefully and accurately. The information you provide on this form and any accompanying documents will be used by the National Lipid Association to determine your eligibility to sit for the examination. If accepted, you will receive an email with instructions on how to register for the exam, and your examination results will be sent to the mailing address indicated on the application.
- You are required to present copies of certificates of attendance for the CME/CE activities that you submit for points and the titles of the activities must be clearly identified. If the title of the session is not clearly related to lipids, we encourage you to submit a copy of the program or educational outline to assist with credentialing review. The Credentialing Committee will make the final determination on point assignments.
- There is no application deadline. Please submit your application at least 3-4 weeks before you plan to sit
for the examination. Once you submit your application please allow up to 2 weeks for processing. Upon approval
of your application, you will receive an email providing instructions regarding selecting your exam date and
time.

Please note: Once a candidate’s application has been approved, they must sit for the exam within the next year or they will forfeit all paid fees and be required to apply to submit a new application and full fee to sit for a future exam.

Eligibility Criteria
Applicants must have achieved a minimum of 10 continuing education credit hours, from a US or Canadian-based accrediting body, in clinical lipidology in the previous two calendar years.
We encourage applicants to satisfy this requirement by completing the National Lipid Association (NLA) Lipid Academy Course. However, credit hours may alternatively be obtained by completing Volume I of the NLA-SAP, the NLA Masters in Lipidology Course or other courses of study in clinical Lipidology.
